Sample Submission Forms

Submitting samples to the laboratory correctly allows us to process your samples efficiently. Here are some tips to assist you in this regard:

• Ensure sufficient sample is provided in the sample bags (500g or filled to the line). 
• Select the correct sample information form (ie Industry Type - Pasture, Cropping, Horticulture, Sugarcane).
• Complete all the relevant details on the sample information form (primarily account number and current test list code). If you require a fertiliser recommendation please fill in the mandatory fields on the back of the forms.
• Ensure the barcode from the sample bag is attached to the sample forms corresponding to the sample details. 
• Place the sealed sample bags and sample forms together in a mail or freight satchel and seal securely.  Please adhere to the kilogram limit found on the satchel.
• Samples should be sent to:

• If you currently do not hold an account with Incitec Pivot Fertilisers and wish to pay with your credit card, please complete and send the Credit Card Payment form with your samples.
